IOP Billing Rules We Handle for New Mexico Programs
Intensive outpatient billing works very differently from a standard therapy visit. Most payers pay IOP on a per diem basis, and each one has its own codes, hour requirements and documentation rules. A small mismatch between your clinical notes and your claim is enough to trigger a denial. Our team knows these rules and builds every claim around them.
IOP Billing Rules We Handle for New Mexico Programs
Since January 2024, Medicare covers intensive outpatient services for mental health. Claims are billed on the UB-04 with condition code 92, and a physician must certify that the patient needs at least 9 hours of services per week under a written plan of care. We check the certification, the plan of care and the weekly hours before any claim goes out.
Commercial and Medicaid IOP Codes
Commercial plans and Turquoise Care MCOs often use HCPCS codes instead of Medicare’s format. We bill S9480 for psychiatric IOP and H0015 for substance use IOP, paired with the correct revenue codes such as 0905 and 0906 where a facility claim is required. We confirm each plan’s preferred code set before your first claim, since the rules vary between Blue Cross Blue Shield of New Mexico, Presbyterian, Molina and UnitedHealthcare.
Prior Authorization and Concurrent Reviews
Most IOP stays need approval before admission and regular reviews to continue. We submit authorizations, track approved days and request extensions on time, so your patients never attend sessions that end up unpaid.
Documentation That Supports Every Claim
Group notes, attendance logs and hours per day must match what is billed. We review your documentation regularly and flag gaps early, including overlapping IOP and PHP days that payers will reject.
Confidentiality for Substance Use Records
Substance use treatment records carry extra privacy protections under federal 42 CFR Part 2 rules. Our billing process keeps these records protected at every step.
